tolua++ exported utility functions missing?

The (out of date?) tolua++ documentation mentions a
series of useful functions which are exported to lua.

However, the latest version of tolua++ does not
contain many of these functions.  In particular, the
tolua.instance() function would facilitate easy OO
programming in lua.

Where have these functions gone?  Were they removed
for some reason?  Is there another way to achieve the
same effect?
